How to Get a Hollywood Fire Rescue EMS Report (And Get Your Bill Sent to PIP)

If Hollywood Fire Rescue & Beach Safety responded to your accident and transported you by ambulance, there's a separate record from the police report: the EMS patient care report. Here's how to request it, who can get one, and how to make sure the ambulance bill goes to your PIP insurance instead of your mailbox.

How to Request a Hollywood EMS Report

Because EMS records are medical records, they're protected under HIPAA — meaning they're handled very differently than a police report.

Option 1: ChartSwap (Electronic) Hollywood Fire Rescue uses ChartSwap.com for medical record requests. Attorneys and authorized requesters select "Hollywood EMS" on the platform to submit a request electronically.

Option 2: Mail-In Patient Records Request You can also mail a completed Patient Records Request form, along with:

• A copy of the patient's government-issued photo ID

• A copy of the requesting party's ID (if different from the patient)

• A Power of Attorney, if applicable

• A copy of the death certificate, if the patient is deceased

Who Can Request It? Only the patient, their power of attorney, or legal guardian can authorize release of an EMS patient care report — this is a HIPAA requirement, not just department policy. Government entities (hospitals, the Medical Examiner, other police departments) requesting records for official purposes go through a separate channel: Fire Rescue EMS Division directly at (954) 967-4248, rather than ChartSwap.

How Long Does It Take?

Hollywood doesn't publish a guaranteed turnaround time for EMS records, but as with most Florida agencies, expect it to take longer than a routine police report request since it goes through a HIPAA-compliant records custodian rather than an open public-records portal. Requests involving a deceased patient (which require a death certificate) generally take longer to process.

Getting Hollywood EMS to Bill Your PIP Instead of You

Hollywood Fire Rescue's ambulance transport billing is handled by a third-party billing company, Change Healthcare:

If you were transported after an auto accident, the ambulance bill should be submitted to your PIP (Personal Injury Protection) carrier rather than billed to you directly. To make that happen:

1. Contact Change Healthcare with your PIP declarations page, policy number, and claim number.

2. Provide the date of service and any incident/run number from the transport.

3. Ask them to rebill or apply the charge to your auto insurance PIP coverage rather than personal responsibility.

There isn't a separate formal "PIP request form" — it's handled through direct communication with Change Healthcare once they have your insurance information on file.

Other Records You Can Request From Hollywood Fire Rescue

Beyond the patient care report, a few other records are worth knowing about:

• Fire Incident or Fire Inspection Reports — requested through the City Clerk's office, separate from medical/EMS records.

• EMS billing statements — for reviewing charges, disputing an amount, or requesting a hardship reduction.

• Run/dispatch records (CAD data) — sometimes obtainable through a public records request to the City for the non-medical portions of a call (arrival times, unit dispatched), useful for building a timeline in a case.
