How to Get your EMS Report After an Aventura Car Accident (And Why It's Not "Aventura Fire Rescue")

If you were transported by ambulance after an accident in Aventura, there's an important detail most people don't realize: Aventura doesn't have its own fire department. Emergency medical services in Aventura are provided by Miami-Dade Fire Rescue (MDFR), out of Station 8 at 2900 Aventura Boulevard. That means your records request goes to the county, not the city.
How to Request Your Aventura EMS/Patient Care Report

Who can request it:
The patient themselves, with valid photo ID
A law firm or insurance company, with a signed, notarized medical authorization
A parent or legal guardian, if the patient is a minor (must state the relationship beneath the signature)
A legal representative of an estate, with a death certificate, if the patient is deceased
Fees: Certified fire property reports and rescue patient reports cost $1.00 per certified page. Because page counts vary, you must call the Records and Resources Bureau for the total before mailing payment (check payable to "Miami-Dade Fire Rescue Department"). Standard public-record copies (non-certified) run 15–20 cents per page.
Submission: Mail your complete request with a self-addressed stamped envelope to MDFR Headquarters, or contact the Records and Resources Bureau directly to arrange delivery.
Why This Record Is Protected Differently Than a Police Report
Your MDFR patient care report is not a standard public record. It's specifically protected under:
Florida Statute §401.30 (the Medical Transport Law), and
HIPAA (the Health Insurance Portability and Accountability Act)
This is a different legal basis than the police report you'd request separately — Aventura PD's crash report is a Chapter 119 public record with a 60-day confidentiality window, while your MDFR patient report is exempt from Chapter 119 entirely and instead governed by medical privacy law. That means the request process, the authorization requirements, and who can access it are all fundamentally different from the police report side.
Billing and PIP: Who to Contact
Miami-Dade Fire Rescue does not charge for on-scene treatment — a fee only applies if you're transported to a hospital by ambulance. Billing is handled by a third-party company:

If your ambulance transport should be billed to your auto insurance PIP coverage rather than to you directly, contact Intermedix directly with your policy declarations page and claim number to have the charge redirected. As with other South Florida EMS billing arrangements, there's no dedicated "PIP request form" beyond providing your insurance information directly to the billing contractor.
What Else You Can Request
Fire property reports — same $1/certified page fee, same records bureau
CAD/dispatch-level incident records — limited, non-medical call data may be available as a standard public record separate from the protected patient report, since the "incident" database and the "medical" database are maintained separately by most South Florida fire-rescue agencies
